Includes: sample selection of form weekly inspection reports in from inspectors giving statistics on inspections, violations and warnings issued in 1940; form application to conduct pharmacy, offering names, address, location, license number, name and certificate number of registered pharmacists, number of hours worked per week, name and certificate number of apprentices, and inventory of store equipment together with affidavits, 1949; and form application for renewal of license, 1941.
